UTILITY SERVICE ACTIVATION
To establish utility service with the City of El Mirage, please complete and submit a Utility Application along with applicable forms, deposits and activation fees to the City of El Mirage Customer Service Division at 12145 NW Grand Avenue, El Mirage, AZ 85335. To request an application by mail, please contact the Utilities Division at 623-933-1228 or by e-mail.
THE FOLLOWING INFORMATION IS REQUIRED ON YOUR APPLICATION:
1. Requested date of connection
2. Property address and tax parcel ID number
2. Applicant and co-applicant information
4. Social Security #, Date of Birth
5. Copy of your valid Federal or State issued photo identification
6. City of El Mirage Business License # (if applicable)
IF YOU ARE:

Owner: Please submit include name of Title Company with a copy of the Escrow # document
- Tenant: Please complete landlord section and submit a copy of the Rental / Lease Agreement
- Management Company: Please submit a copy of the Management Agreement

Realtor: Please submit a copy of the Listing Agreement

Deposits will be applied toward the final bill and the applicable refund will be mailed out after termination of service. Please allow two (2) business days (excluding holidays) from the application date after all applicable forms have been received in our office for service to be activated.
Note: Only completed applications with all required documentation will be accepted.
SERVICE TRANSFERS:
If for any reason a customer needs to transfer an account, a Termination Application must be completed for the property you are vacating and a Utility Application must be completed for the new service location.
DISCONTINUANCE OF SERVICES UPON REQUEST OF CUSTOMER:
Any Person who desires to disconnect the use of services a Termination Application shall be filled out and submitted at least three (3) business days (excluding holidays) in advance of intended discontinuances of service.
